Patient Policies

Dr. Dietrich is committed to providing patients with timely, quality care. In order to do so, we request that you arrive on time and make every scheduled appointment. We feel it is important for you to understand our policies as they relate to cancellations, no shows and late arrivals.

New Patients

If you are new to our practice, we ask that you arrive at least 30 minutes prior to your appointment time and have your new patient forms completed. This early arrival allows our staff to efficiently perform its task of registering you into our systems.

To ease this process, click on the “Patient Forms” tab to access forms you can download, print and fill out prior to your appointment.

Existing patients

If you require X-rays at your next appointment, please arrive 30 minutes prior to your visit to have those taken at our office.

Follow-Up Visits

For follow-up visits, we ask that you arrive on time. Should you arrive more than 15 minutes late, your appointment may be moved to a work-in status or rescheduled.


If you have a scheduled appointment time and need to reschedule or cancel, please notify our office at least 24 hours prior to your appointment. We will gladly reschedule you a more convenient appointment time. No shows and cancellations made less than 24 hours before a scheduled appointment time may be subject to a cancellation charge.


All emergencies are seen in a timely fashion, based upon the urgency of your problem. In a true emergency, it is best to go to the emergency room at your nearest hospital. The emergency room staff will contact us or the physician on call. You have the right to request direct care from our physician when in the emergency room.

Medication Refills

Please call the office during normal business hours for prescription refills and have your pharmacy fax your medication refill request to our fax line, 817-801-1508. Please allow 24-48 hours for us to complete your request.

Fees and Insurance

Insurance co-payments, co-insurance and deductibles will be collected at the time of service. If you are a self-pay patient (uninsured), we require payment in full at the time of services. If you are unable to do this, please call our office to discuss payment arrangements. A discounted service fee will be extended to all self-paying patients who pay in full at the time of service.
